Role Overview
We are building a proprietary healthcare platform from the ground up—without relying on existing EHR or patient record solutions. The goal is to create a unified patient operating system that provides a holistic, longitudinal view of each patient and enables predictive insights into who will need care and when.
This role is critical and immediate. You will take ownership of an early-stage platform currently developed by an external dev shop, identify technical gaps, and drive the architecture forward until an internal engineering team is established. You will partner closely with a product-led founder who has defined the strategy and roadmap and needs a senior technical counterpart to execute, challenge assumptions, and shape the system design.

Key Responsibilities

Take over and advance an existing dev shop–built codebase, including evaluating architecture, identifying gaps, and improving scalability and reliability.

Design and build a custom patient record system from scratch (no off-the-shelf healthcare platforms or EHRs).

Architect the core patient operating system to support a holistic, longitudinal view of patient data.

Build foundational data models to support predictive care use cases (who needs care, and when).

Lead backend development on Google Cloud Platform (GCP) using healthcare-appropriate services.

Design and implement FHIR-based data models to support patient lists and core clinical entities.

Build and maintain integrations with third-party systems, including telephony (RingCentral).

Own backend infrastructure including Firebase, BigQuery, and GitHub Actions for CI/CD.

Collaborate with product leadership to translate product strategy into technical execution and clearly articulate technical risks, gaps, and tradeoffs.

Establish engineering best practices, documentation, and standards in preparation for future hires.

Maintain control over the application UI architecture while remaining primarily backend-focused.

Required Qualifications
-
10+ years of professional software engineering experience, with senior-level ownership of backend systems.

Strong hands-on experience with Google Cloud Platform (GCP) — this is a must-have.

Healthcare experience with FHIR data models and patient record systems.

Experience building custom healthcare platforms or patient databases from scratch.

Strong backend engineering background; capable of full-stack work but not frontend-focused.

Experience taking over externally built projects and driving them forward independently.

Comfortable working in ambiguity and making foundational architectural decisions.
Preferred Qualifications
-
Prior experience in early-stage startups or zero-to-one platform builds.

Experience with predictive analytics or data modeling in healthcare.

Integration-heavy system experience (telephony, third-party APIs, data pipelines).

Experience working closely with product-led founders or leadership teams.
